The following steps must be completed in order to be included in the Consortium:

You will need to review the Policy. If you want any additional information in the Policy that is not included, fax (217) 525-0342 or email shellym@midwesttruckers.com the additional information. If the Policy is completed to your satisfaction, complete the Policy effective date, contact names, office location, day and nighttime telephone numbers and make a copy for each employee to keep.

Make 2 copies of Attachment G (Acknowledgement of Drug & Alcohol-Free Workplace Policy), to be completed by each employee. One copy will be kept in the employer's file, the other to be returned to the employee.

If you have chosen to pre-employment test an applicant, you will need to have the applicant complete Attachment A (Pre-Employment Notification and Acknowledgment Form) in addition to the Attachment G.

You will need to complete Attachment X (Employee Action Form) in its entirety and send it to: Midwest Service Corporation (MSC), 2727 N. Dirksen Parkway, Springfield, IL 62702, fax it to (217) 525-0342, or email it to Shelly (email addresses in #1 above). WE DO NOT REQUIRE YOU TO SEND US COPIES OF THE DRUG AND ALCOHOL-FREE WORKPLACE POLICY OR ATTACHMENTS A & G. Those copies will be kept on file with your records (NOTE - We will not accept any other type of form to add employee(s) into the Consortium. Only Attachment X will be acceptable.)

Once Attachment X is received by the Consortium, the pre-employment tests will be scheduled if you have chosen to conduct pre-employment or pre-enrollment testing on your applicants/employees. You will receive a phone call from our collection company representative to confirm all the details (day, time & place) for the collection if you chose onsite testing. If you chose clinic-based testing, provide the forensic custody and control form to the applicant/employee and send him or her to the clinic you had designated to Midwest Service Corporation you would to use for testing.

If you have chosen to random test your employees, we will enter your employees into the random pool. The Consortium's service agent will conduct random selections on a regular basis. When you receive a random test notice from the Consortium, you must complete the required tests before the end of the random selection period. No employee should be informed of the random test(s) except just prior to the actual test(s) being performed.
All drug & alcohol records shall be maintained in a secured location with controlled access. All negative drug test results and alcohol test results of less than the alcohol concentration level you selected (.02, .04 or .08) must be maintained in your file for a period of one year. All positive drug test results, alcohol test results at the alcohol concentration level you selected (.02, .04 or .08), and refusals to test shall be maintained for five years.
All other records concerning the drug & alcohol program shall be maintained per the employer's practice.
If you have chosen to conduct post-accident/incident tests on employees, and a post-accident/incident drug and alcohol test needs to be conducted, Attachment C (Employer's Record of Post-Accident/Incident Test) should be completed and kept with your records. You should instruct each employee on the procedures to follow when an accident/incident occurs (see Attachment D - Instructions to Employee Involved in an Accident/Incident). It is your responsibility to let the employee know when a post-accident/incident drug and alcohol test is required. It is also your responsibility to call the Consortium to get the post-accident/incident test completed as soon as possible after the accident/incident occurs. The Consortium is on call 24 hours a day, 7 days a week at (217) 525-0310 to handle any post-accident/incident tests.
If you have chosen to conduct reasonable suspicion tests on employees, a reasonable suspicion test needs to be conducted when you suspect an employee is under the influence of drugs and/or alcohol, which would be reflected by unusual actions (see Section 4 - Reasonable Suspicion Testing). Your company's designated person or people who have been trained should complete Attachment E (Reasonable Suspicion Observation Form) within 24 hours of the observed behavior or before the drug test results are released, whichever is earlier. It is your responsibility to call the Consortium to get the reasonable suspicion test completed as soon as possible after your designated person or people have observed the employee to determine reasonable suspicion. The Consortium is on-call 24 hours a day, 7 days a week at (217) 525-0310 to handle any reasonable suspicion tests.
All persons designated to supervise the employees must complete training on alcohol and drug abuse as per the employer's practice. Training needs to be conducted only once, but periodic reviews are recommended. Records related to the education and training of the designated persons should be maintained by you for their term of employment and two years thereafter.
We must be informed immediately when you delete an employee. Attachment X (Employee Action Form) must be completed and faxed or mailed to us. The employer is not in compliance with their drug & alcohol-free workplace program when their list of employees is not current.
We will bill you for whatever type of testing you choose to conduct. Also, if an employee/applicant requests a retest, you will be billed for that test which was conducted on your employee/applicant's behalf. See the enclosed price sheet for Consortium charges. Please pay in full promptly upon receipt of invoice.
